Majicwell Enterprises Inc., 101 N. Maple St., Bainbridge, Jan. 7. Violations: Ham and other time and temperature controlled for safety foods in the cold-holding unit were at 46 degrees. The thermometer in the cold-holding unit was not functioning properly.

Bainbridge Shell, 101 N. Quarry St., Bainbridge, Jan. 7. Violations: Milk in the refrigerator was found at 46.5 degrees (critical violation). An English muffin was an egg was not properly labeled (critical violation). A test kit was not available to verify concentration is the solution.

Quick One Stop, 35595 Ohio 35, Richmond Dale, Jan. 8. Violations: An accumulation of a mold-like substance was found on the pop nozzles. An accumulation of debris was found under the food prep tables in the kitchen, under and behind the cooking equipment in the kitchen, under racks in the walk-in refrigerator and on the racks stocking milk in the walk-in refrigerator. There was a buildup of debris around the openings of the air outlets on the ceiling.
