An accumulation of debris was found under the food prep tables in the kitchen, under and behind the cooking equipment in the kitchen, under racks in the walk-in refrigerator and on the racks stocking milk in the walk-in refrigerator. There was a buildup of debris around the openings of the air outlets on the ceiling.
